What Do You Need to do to Set Up an Appointment?
- Call Pratt County Health Department to make an appointment at 620-672-4135.
- Come with your child to their appointment.
- Call if you cannot keep your appointment.

- Nutrition services
- Injury prevention
- Substance use (alcohol & tobacco) prevention
- Lead poisoning prevention
- Addressing childhood abuse and neglect
- Screening services including vision, hearing, developmental, scoliosis, urine, hemoglobins, & blood sugars.
In addition, local agencies are encouraged to take a leadership role in developing comprehensive systems of child health services in their communities. Examples of services provided are:
- The health department staff will assist clients in filling out the applications for Healthwave and Medicaid.
- Integration with other health and social services such as immunizations, WIC, oral health, teen pregnancy prevention, sexually transmitted disease prevention and treatment, and women's health.
